People ask "how many units is 250 mcg?" as if there were one answer. There is not, and the reason is the single most useful thing to understand about a syringe: units measure liquid, not substance.

What a unit is

On a U-100 syringe, 100 units is 1 mL of liquid. A unit is 0.01 mL, whatever is in it. Water, saline, a mixed vial: a unit is a unit of volume.

What sets the substance per unit

How much powder was in the vial, and how much water you added. 5 mg in 1 mL gives 5 mg per mL, so 50 mcg per unit. The same 5 mg in 2 mL gives 25 mcg per unit. Same vial, same syringe, half the amount per line. That is the whole reason the reconstitution calculator asks for the water volume first.

Why this catches people

  • A number remembered from a forum was for someone else's water volume.
  • A vial was topped up with more water and the label was not updated.
  • Two vials of different strengths were mixed with the same water and treated the same.

The habit that fixes it

Write the water volume on the vial with a label the moment it goes in, and work every draw from that number with the calculator. The reconstitution term page and the units guide go deeper.
