How to Pack a Medication Cooler
The night-before routine, the layers from the pack up, and why the vials never touch the ice: how to pack a medication cooler so it actually holds.

A cooler fails in three ways: it was never cold to start with, the contents froze against the pack, or it was opened so often that it warmed anyway. Packing it properly is ten minutes the night before. This is the equipment side; the flying guide covers what security in each country expects.
The night before
- Freeze the packs flat for at least 12 hours; a pack that is "mostly frozen" lasts half as long. For an evaporative wallet, soak it for the time on the label and blot the outside dry.
- Chill the cooler itself: put the empty cooler in the fridge or drop a spare pack in it. Insulation only slows change; a warm cooler starts by warming your vials.
- Chill the vials: they go in straight from the fridge, never from the counter.
The layers, bottom to top
- Frozen pack, flat.
- A folded cloth or the cooler's divider. Vials in direct contact with a frozen pack can freeze, and a frozen vial is a ruined vial.
- Vials in their case or amber bag, standing up, with the logger or thermometer beside them.
- Second cloth, second pack if the cooler takes one.
- Syringes, pads and the travel sharps container on top or in the outer pocket: they do not need to be cold and they are what security wants to see first.
On the day
Open it as little as possible; every opening costs about an hour. Keep it out of direct sun and off the car floor over the exhaust. If there is a fridge at the far end, transfer straight away and re-freeze the packs. Read the logger before you throw the packaging away; if it shows a warm spell, the temperature-loss calculator tells you what it cost.
How long does it actually last?
The product records give a figure for each cooler (about 24 hours for a good ice-pack cooler, about 45 for an evaporative wallet, a day on battery for a powered one), measured in a room, not a hot car. Halve it for a hot day; double the packs for a long one. For anything over two days, the powered cooler comparison is the honest read.
